First appointment

We’ll ask you to fill out a health questionnaire at your first appointment. To save time, you can complete the questionnaire at home, print it out and then bring it to your appointment. Rest assured that all information is kept strictly confidential.

If you need to cancel an appointment, please call or send us an email at least 48 hours in advance. Patients will be charged for missed appointments or last-minute cancellations.
